# Transcripts

To request an official transcript, complete the [Document Request Form](https://mountsinai.formstack.com/forms/document_request_form) located on the [Registrar’s Office website](https://icahn.mssm.edu/education/students/registrar). Students may print an unofficial copy of their transcript for from the Empower Student Information System. Unofficial transcripts cannot be used for official purposes.
